I found one continuity error from the latest episode "316" that I didn't see posted on your site yet. When Jack asks if Kate still drinks her coffee with milk and two sugars, there is definitely no milk carton on the table. Kate sits down and Jack goes to his kitchen. While they are chatting about Jack's dad (Jack is still in the kitchen) you can see a tiny part of a milk carton suddenly show up near Kate's arm. When Jack returns to the table, the red milk carton is clearly visible.
